For men stable on testosterone therapy.

Includes:

• 1 hormone follow-up visit per  quarter (in-person or telehealth)
• Testosterone dose monitoring  & adjustment
• Review of labs (testosterone,  estradiol, hematocrit as indicated)
• Prescription management &  refills
• Secure messaging for hormone-related questions
• Visits are hormone-related only

• Active dose optimization
• Lab review and trend tracking
• Prescription management &  refills
• Secure messaging with clinical  team
• Lifestyle and symptom  optimization guidance
• Visits are hormone-related only

Not included:

• Labs (low- cost cash labs are available if insurance is not available)
• Medications
• Pellets or injection procedures
