No. At Restore Health & Wellness, LLC we do not accept insurance. Most treatment options used are not covered by insurance. We are happy to provide you with an invoice so you may submit it to your insurance company. Labs may be covered by insurance, but you will need to submit these on your own. 

Weight loss patients: After the initial visit we typically follow up 6 weeks later to address any questions or concerns you may have after starting treatment. Typically after that we follow up every two months.

Hormone therapy: We will typically follow up every 6-8 weeks until labs are at stable and as needed.

Yes. we will accept outside labs from your PCP and these can be uploaded into the EHR when you book your appointment and register with the portal. We use a company called Access Medical Labs, that will send a lab kit to your house and you can go to any Labcorp or Quest to have these drawn for $10. We can also have a kit sent to you that will allow you to collect your specimens at home and ship back to the lab. We ask if you send labs from your PCP they are no older then 6-months.  Please be aware we require labs prior to starting and medical treatment. 

We can also order a panel of labs after your first visit through Quest labs. We would like fasting labs completed as soon as possible. 

Once you book your appointment and we confirm you will be sent a package of forms you can fill out and sign via your phone or computer. We ask that these are completed prior to your appointment so we have time to look over your paperwork before the initial visit.
