No. At Restore Health & Wellness, LLC we do not accept insurance. Most treatment options used are not covered by insurance. We are happy to provide you with an invoice so you may submit it to your insurance company. Labs may be covered by insurance, but you will need to submit these on your own. 

Weight loss patients: After the initial visit we typically follow up 6 weeks later to address any questions or concerns you may have after starting treatment. Typically after that we follow up every two months.

Hormone therapy: We will typically follow up every 6-8 weeks until labs are at stable and as needed.

Yes, we accept outside labs from your PCP (within 6 months). Labs can be uploaded through the patient portal before your appointment.

We also offer discounted lab options through Access Medical Labs with at-home kits or draws at Labcorp/Quest ($10 draw fee). Quest labs can also be ordered after your visit.

Labs are required prior to starting treatment, and fasting labs are recommended.

Once your appointment is confirmed, you’ll receive forms to complete electronically. Please submit them before your visit so we can review your information in advance.
