 Initial evaluation, hormone balancing and medical monitoring throughout treatment.
➢ Personalized dosing and ordering of hormone medications by mouth or topical, OR  at-home dosing of weekly administered TRT injections.
➢ Initial and follow up lab (2 lab draws yearly minimal and as needed). 

Option 1: $350.00 for initial visit (includes labs, visits and medications). 

Additional medications be added such as clomid or HCG, these medications will be charged separately.

$175 per month

Monthly membership payments of $175.00 will be due on the 1st of each month until your membership is terminated in accordance with this agreement.
