Direct Primary Care (DPC)- Coming Soon

Our Tier 1-Direct Primary Care memberships include in-person and virtual office visits consultations with the medical provider, basic labs, preventative care, wellness visits, chronic disease management, sports physicals. 

Our Tier-2 Direct Primary Care membership includes  all of Tier-1 Services as well as Cologuard, DEXA scans, CT calcium Score and Mammogram. 

 Our DPC memberships do not cover care provided to members in other locations, such as the hospital, specialist care (such as oncology, cardiology to name a few), an outpatient surgery center or at an urgent care or general diagnostic imaging. Additional X-Rays, CT scans etc will be at an additional cost to the patient at a discounted cash price. 

Yes. It is recommended to have an additional health insurance plan.

 Direct primary care memberships give patients access to a variety of primary care. But people still need health insurance in case they end up needing medical care that goes beyond the basic primary care services available through a DPC membership.

The DPC membership will allow unlimited access to a primary care provider who can take care of the patient's basic needs. And the health insurance plan will be there in case more significant or specialty care is needed.

DPC- Tier 1: cost $79 month- each additional family member (18 and under)  + $25 month

• Sick visits 
• Basic lab panels 
• Direct contact with a provider 
•  Chronic conditions
•  Preventive care

   Additional screenings not included: 

Mammogram, Cologuard, DEXA scan and CT calcium score are paid outside of the monthly fee at a cash price
